The Francophone Community Health Centre Hamilton/Niagara– in French,
the “Centre de santé communautaire”- was established in
1992. Our Centre is a multi-service agency providing health and social services
to French-speaking clients at our two sites: in Welland, serving the Niagara
region, and in Hamilton serving the new city of Hamilton.
Programs
and services
Our programs and services have a particular emphasis on prevention and
health promotion achieved through the provision of information and education.
Services vary slightly between Hamilton and Welland according to the needs
and resources in each community.

Program for primary health care: a medical clinic with a family doctor,
several nurses and a nutritionist

Pre and postnatal program: pre/postnatal courses, childbirth and breast
feeding support, nutritional support, accompaniment

Promotion of health : workshops, documentation and display units relating
to a variety of subjects connected to health and the well-being of everyone

Programs
and support services for families:
- Good beginning, Brighter Future, Fathers’ Group: these programs
group a diversity of services and activities to help families with children:
home visits, groups support, recreational activities for children, collective
supper, various workshops on health promotion and development of parental
competences, nutritional and material support
- Coin de la famille: a resource center for children 0-6 years and parents
(books, vidéos, CDs, toys), educational and recreational activities
- Clothing exchange
- Individual and family counselling
- Accompaniment and referrals to external resources.

Support groups for women (in collaboration with the Sexual Assault Women’
Centre of Hamilton– Outreach Program):
- Immigrant women support group
- Other groups and activities depending on funding

programs for seniors (50+):
- Let us be Active (physical exercises)
· Foot care
· Community nursing services
· Health promotion: workshops
· Community dinner
· Individual counselling and support
· Volunteer led educational/social/recreational programming
· Transport services
· Collaboration with other organizations to facilitate access to
other services (Teletouch, Friendly visiting program at Family Services)

Program and services for newcomers:
- Settlement and integration services: support, referrals and accompaniment
(housing, social and health services, school registration, employment and
education, legal immigration issues)
- Legal clinic (refugee issues)
- Workshops on different issues
- Fathers’ group
- Club de l’amitié: Volunteer led socio/recreational activities
(monthly meeting and supper, soccer team, outings, etc.)
- Arc-en-ciel: program for children and their parents victims or witness
of war and torture.

Anti-violence Programs: prevention/education and intervention programs
for children, teenagers, adults and older people:
· Pre-school program for the prevention of aggressions
· School age programs ESPACE and RIPOSTE
· Anti-bullying program for school children: L’intimidation:

· Self-defence course INSTINCTS for girls and women
· Individual support for children witnessing violence
· Transition support program for women victims of violence
· 24 h/7 days phone line “elle-écoute” (in collaboration
with regional Francophone agencies)
· Regular workshops on different issues related to violence
· Screening of abuse (nurses and doctor)
· Individual counselling
· Legal clinic (family law)
· Referrals to external resources.

The majority of our services are provided on-site, but we also work in various
locations in the community:

Client’s homes: home visiting services to families in need and
friendly visiting services to people living in isolation

We accompany clients to social services (Welfare, Housing, etc.)

Programs and workshops in schools, churches and community centres
Our services are free of charge. We do our best to provide services promptly,
but in some cases, we have a waiting list or a waiting period. If we cannot
provide a particular service, we do our best to connect people with appropriate
services available elsewhere in the community. In some cases, we work closely
with an outside agency to ensure that services are available in French.
Partnerships
and collaboration
The Francophone Community Health Centre Hamilton/Niagara strives to work
with outside agencies to help increase the range and quality of services for
French-speaking clients (e.g.: shelters, early childhood centres, settlement
and integration services, home visits for seniors, etc.).
